Department of Motor Vehicles
You may qualify for additional resources due to your impaired mobility due to having lost use of one or more lower extremities, or both hands, or have a diagnosed disease that substantially impairs or interferes with mobility, or one who is severely disabled to be unable to move without the aid of an assistive device.
Employment Development Department
The EDD manages the Unemployment Insurance (UI) and State Disability Insurance programs which include Disability Insurance (DI) and Paid Family Leave (PFL) for the State of California. You can file a claim for either of these programs online, by phone, or by mail.

When you come for your appointment, please bring the following
- Driver’s License or a valid ID
- Insurance information
- Referral Letter (if required)
- Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
- List of medications (if any)

Telephone the office during business hours and allow at least 1 days' notice so that we can offer your appointment time to patients on our waiting list.
We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
